You can add a form to your site with the Contact Form widget or the Advanced Forms widget (Editor 2.0), or by asking Copilot in a Vibe project. This article covers what applies to every form: collecting consent, responses and email notifications, and health information.
To build or set up a specific form, see its article:
To get a visitor's consent when they submit a form, add an Opt-in field: a checkbox with consent text the visitor checks. Keep a marketing consent checkbox optional, so visitors can submit the form without agreeing to marketing.
To add an Opt-in field:
-
Contact Form widget: see Widgets: Contact Forms.
-
Advanced Forms widget: see Widgets: Advanced Forms.
-
Vibe project: forms don't include a consent field by default. Ask Copilot to add one, for example, Add a checkbox to the contact form asking visitors to opt in, and set it to not required.

Every form submission is saved in your form responses, so you can still see it if an email notification doesn't arrive. Submissions sent from a preview link aren't saved and aren't included in the CSV export.
Each form can email the site owner when someone submits it, and send an automatic reply to the visitor. To find responses and set up these emails, see the article for your form type above.
If notification emails aren't arriving, check that the recipient addresses are correct, then see Troubleshoot Missing Site Emails.
